Registered Nurse (RN) Home Care Case Manager

Location: Greater Springfield, MA Area
Salary Range: $90,000 - $100,000 per year
Job Type: Full-time

Key Responsibilities:

  • Manage a caseload of home health patients, coordinating and delivering personalized care plans.
  • Conduct initial and follow-up assessments to identify the physical, psychosocial, and environmental needs of patients.
  • Develop, implement, and evaluate individualized care plans in collaboration with interdisciplinary teams.
  • Maintain clear and timely communication with the Clinical Supervisor, physicians, and other team members regarding any changes in patient status or care needs.
  • Ensure patient confidentiality and compliance with HIPAA regulations.
  • Document all care and communication according to policy and procedure.

Qualifications:

  • Current, unrestricted RN license in Massachusetts.
  • Minimum of 1 year of experience in a clinical care setting.
  • Prior home health or hospice experience is preferred.
  • Reliable vehicle, valid driver’s license, and current auto insurance.
  • Ability to travel 50% within the service area.
  • Associate's or Bachelor’s Degree in Nursing preferred.
  • Familiarity with HomeCare HomeBase is a plus.
