Position Overview
As a Home Health RN Case Manager, you will coordinate and direct patient care based on individual needs. This role involves independent management of home health patients, utilizing advanced assessment, teaching, and decision-making skills. You will play a key role in interdisciplinary collaboration, ensuring patients receive comprehensive, high-quality care.
Key Responsibilities
Perform comprehensive assessments of patients' physical, functional, psychosocial, and cognitive status.
Develop and implement individualized care plans in collaboration with patients, families, and physicians.
Provide skilled nursing care, rehabilitative procedures, and prescribed treatments in a home setting.
Educate patients and caregivers on self-care techniques and medication management.
Monitor patient progress, adjust care plans as needed, and ensure proper documentation.
Coordinate with interdisciplinary teams and community resources to support patient care.
Maintain compliance with home health standards and regulatory guidelines.
Participate in quality improvement initiatives and contribute to professional development efforts.
Serve as a resource for clinical concerns and mentor other healthcare professionals as needed.
Take part in on-call responsibilities and travel within the assigned geographic area.
Qualifications
Licensure: Active Registered Nurse (RN) license in the state of Wisconsin.
Education: Bachelor's degree in Nursing (preferred).
Experience:
Minimum one year of medical-surgical clinical nursing experience required.
Previous experience in a Medicare-certified home health agency preferred.
IV therapy skills and experience with venous access devices and ambulatory pumps.
Proficiency with home health medical record software, such as Epic, is a plus.
Certifications: Current CPR certification required.
Other Requirements: Valid driver's license and reliable transportation with current auto insurance.
