Job Summary
Make a meaningful impact in your community—join Acentra Health as a Clinical Evaluator (PASRR) in Manhattan, Kansas. In this hybrid role, you will conduct in-person preadmission evaluations and assessments throughout Manhattan and its surrounding areas.
As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. . These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.
This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. Travel expenses—including mileage and parking are covered and reimbursed by Acentra Health. In addition, Acentra provides all necessary technology equipment for your home office, including a laptop for use in the field.
Job Responsibilities
As a Clinical Evaluator. you will be conducting PASRR Level II assessments and screenings and evaluations, developing informed level-of-care recommendations based on your assessments.
Conduct assessments in various environments, including nursing homes, hospitals, and community settings, to understand and address the diverse needs of those we serve.
Adhere to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports.
Ensure compliance with contractual and regulatory standards while developing level of care recommendations
Utilize independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities to navigate complex situations, ensuring the best outcomes for members, providers, and clients.
Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements.
Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.
Stay informed on current literature, regulations, and research pertaining to long-term care, mental illness, medical conditions, and geriatrics.
Execute tasks with strong attention to detail, ensuring adherence to established workflows to maintain high standards of quality and operational efficiency.
Qualifications:
Required Qualifications, Skills and Experience
Must possess an active, unrestricted license in the State of Kansas in one of the following fields: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Psychologist (LCP), Registered Nurse (RN), or another qualified Licensed Mental Health Professional (LMHP).
Graduation from an accredited college with a relevant degree program and minimum of 2 years of experience assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.
Working knowledge of Preadmission Screening and Resident Review (PASRR) requirements, including applicable Federal Regulations and state-specific PASRR rules.
Skilled in assessing and analyzing psychological, emotional, behavioral, and physical functioning to develop clinically appropriate level of care recommendations.
Knowledge of standards of practice for determining Level of Care, including best practices in mental health, geriatrics, and conditions commonly requiring long-term care placement.
Familiarity with psychiatric disorders as outlined in the current Kansas Diagnostic and Statistical Manual (DSM).
Excellent verbal, written, and electronic communication skills with the ability to engage effectively with individuals and groups.
Detail-oriented with strong organizational skills and able to prioritize and complete tasks efficiently within set timelines.
Familiarity with public mental health and medical programs.
Proficient in computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams).
Previous experience working with the geriatric population is preferred.

Compensation
The compensation for this role is $37.00 to $43.00 per hour
Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
