What You'll Do:
Empower Patients: Provide comprehensive care management to patients with chronic kidney disease, helping them navigate their healthcare journey.
Ensure Continuity of Care: Facilitate seamless transitions between inpatient and outpatient settings, utilizing available resources and patient health plan benefits.
Provide Telephonic Support: Virtually meet with patients, families, and caregivers to discuss care and treatment plans.
Identify and Assist High-Risk Patients: Proactively identify and assist high-risk patients in acute care, skilled nursing, and ambulatory settings.
Collaborate with the Care Team: Consult with physicians and other team members to ensure successful implementation of care plans.
Coach and Motivate Patients: Utilize motivational interviewing and behavioral change techniques to maximize patient self-management.
Coordinate Discharge Planning: Oversee discharge planning, including follow-up appointments, home health, and social services.
Maintain Accurate Documentation: Document patient information and care management plans in our electronic health record system.
Work Flexible Hours: This position includes some evening hours to accommodate patient availability.

What We're Looking For:
Active RN License: Active RN license in your current state of residence, with the ability to obtain additional state licenses as needed.
Nursing Experience: 2+ years of nursing experience in a hospital, acute care, or direct care setting.
Renal Care Experience: Experience in renal, chronic kidney disease, or dialysis care.
Computer Proficiency: Proficiency in MS Office (Word, Excel, PowerPoint, Outlook) and the ability to navigate a Windows environment.
Home Office Setup: Access to a dedicated workspace and high-speed internet connection.
Flexible Schedule: Ability to work some evening hours, as needed.
Preferred: BSN, Certified Case Manager (CCM), diabetic educator experience, ICU, cardiology, or critical care experience, telephonic case management experience, discharge planning experience, and knowledge of hypertension and/or diabetes.
